Community Work
Our Community staff visit hospitals and rehabilitation centres to provide extra information and support at every stage along the care pathway. They also make home visits and can accompany clients to appointments and interviews. The aim is to help each person find their next step in skill development and independence. Case Worker - works closely with Oxford Centre of Enablement. Offers support to people with complex needs requiring the help of more than one organisation. For instance a brain injury accompanied by a substance misuse problem, or a housing issue, or financial management difficulties. Support Worker - makes weekly visits to the John Radcliffe hospital including the Neuropsychology Dept, West Wing, and provides information on a wide range of topics from the physical and emotional effects of brain injury, to care and social options after discharge. Refers on for specialist help with benefits. Makes home visits and liaises with Community Hospitals. Cherwell District Support Worker - liaises with users of Leonard Cheshire Day Opportunities and provides information and support to other residents of Cherwell District affected by brain injury. Vocational Worker - liaises with Oxford Centre for Enablement and provides support on the road back to gainful employment either to a previously held position or to something new.
